Policies:

Accepting new patient, starting at 2 years of age. Must be current with all vaccines.

New patients requesting treatment for adult onset ADHD, will not be treated with stimulant medications. Other alternative treatment will be offered. If more aggressive treatment is required you will referred to an outside specialist.

Workers Comp accepted. Please have your employer contact office prior to scheduling. All paper work and approval must be completed before scheduling. See forms below.

We provide initial treatment for pain, depression and anxiety. Chronic care for these ailments may be referred out to a specialist, at the discretion of our providers.

Most commercial insurances accepted. Please call to verify. Please be aware of your policy and your coverage. You are responsible for all co-insurances, deductibles, balances and non-covered service fees. Please have insurance card with you for every visit.

All Copays and past due balances are due at the time of service.

We do provide most childhood vaccines as well as offer vaccines for adults. Please call in advance to schedule the appointment to ensure there is adequate stock. (We do not provide flu or shingles vaccine at this time.)

Patients must be seen every 6 months in order for your provider to maintain prescriptions. Many medications will require blood work on a biannual basis.

Lateness – Please be aware that if you are more than 15 minutes late to a scheduled appointment, you may be asked to reschedule to another time or date.

Returning patients should arrive 5 minutes prior to your appointment time. New patients, please arrive 10 minutes prior to your appointment to complete all new patient paperwork. All patients are expected to complete check in sheet upon arrival. Forms provided at bottom of web page.

We can perform annual physical examinations and will verify benefits prior to the date of the appointment. Please ensure our office is aware of any changes to your insurance plan. Please note that if any medical issues are addressed during your visit (colds, medication refills, etc.) a medical visit will be billed along with the physical. Your co-pay will be required at the time of the visit.

We do have an after-hours answering service for emergencies not requiring 911 services. Please be aware that we are unable to call in medications or handle any routine issues after business hours. If your issue can wait, please call our office Monday through Friday between 7am and 5pm so that we may address the issue appropriately.

Insurance:

Not accepting new Medicare patients.

Not accepting TennCare patients.

Please call to check if your specific plan is accepted. There are daily changes that we are not aware of from insurance companies. Always be aware of your policies changes.
